It seems that Ghost escaped death after being shot  by General Shepherd when his team from Tasks Force 141 raided Makarov's hideout to retrieve some vital intelligence in COD MW2. He has become disillussioned while working for ActivistSun, the PMC company left him alone and had other plans, and his mates were killed and Soap, Price and Nikolai are nowhere to be found. What Ghost then finds out that ActivistSun is now branded a terrorist organisation by the United Nations having joined Makarov, and FOXHOUND team leader, Liquid Snake/Ocelot. They are bent to build a weapon called "Modern Gear" for world domination. Ghost was determined to find out what happened and find any survivors of his team by going after ActivistSun.

For Solid Snake,  the discovery of Liquid Snake  joining  ActivistSun and Makarov  forced him to go to the Philanthropy Headquaters where he was a given a mission by Otacon to take down the new menace. He was mistaken that he had vanquished Liquid, after their hand-to-hand combat in which he won in Metal Gear Solid 4: Guns of the Patriots,

Meanwhile, Ghost infiltrated the ActivisitSun Headquarters only to be captured by Liquid. He was rescued by Solid Snake when he was being fed information by his guard before he gets executed. Solid Snake brings Ghost to Philanthropy where they were ordered by Otacon to team up and proceed with the mission to destroy the "Modern Gear."
